The Cibola General Hospital Health Information Management Department maintains a confidential record consisting of your diagnosis, treatment, and progress for each of your visits. This information details the chronology of your care and gives physicians and other medical specialists the background they may need to ensure the best possible outcome and continuity of care. You have the right to review and obtain copies of your health records for a small fee.
The Health Information Management Department is open Monday through Friday from 8:00 AM to 4:30 PM. You may contact them at 505-287-5242 during business hours with any questions you may have regarding the release of information process or other medical records needs.
In-Person
Come to our office at 1010 E. Roosevelt, Grants, NM 87020 to file an in-person Release of Records Authorization Form. We are open
from 8:00am – 4:30pm, Monday – Friday. We are closed on holidays and weekends.
