Our Family Health Center is located at 1423 E. Roosevelt Ave., Grants, NM 87020, just half a mile (1-minute drive) from the main hospital campus. Here, we provide primary care for people in Grants and nearby communities from 8 a.m. to 5 p.m. Monday through Friday. Within this center, we also have our Acute Care Clinic, which provides urgent care services Monday through Saturday from 8 a.m. to 7 p.m.
Cibola Specialty Associates is located next to the main hospital campus at 1208 Bonita Ave, Grants, NM 87020. This clinic specializes in orthopedic, cardiology, and podiatry care, providing our community with expert-led treatment that focuses on alleviating pain and improving long-term mobility. Visit us from 8 a.m. to 5 p.m. Monday through Friday.
Our Cibola Professional Services building is located at 1010 E Roosevelt Ave, Grants, NM 87020.
In Suite A of this building, we offer behavioral health and wellness care such as counseling and therapy from 8 a.m. to 5 p.m. Monday through Friday.
Suite B is our Billing and Medical Records office, which is open from 8 a.m. to 4:30 p.m. Monday through Friday. This is where patients can get assistance with billing questions, insurance inquiries, payment arrangements, and medical record requests.
