Nursing Case Management
The Case Management department at Cibola General Hospital assists patients and their families with many different aspects of care, beginning when a patient is admitted to the hospital and continuing through discharge.  The Nursing Case Manager works to ensure the best possible care is received and the best possible outcome is achieved. The RN CM works daily with all patients insurance companies and assists with coordination of discharges home, into a short or long term facility or transfer to another hospital if needed.

Discharge Planning

This process begins the day a patient enters the hospital and doesn’t end until the patient is back at home and comfortably settled in. Discharge planners arrange for everything from durable medical equipment, in-home care services, physical rehab, home oxygen, Meals on Wheels, and specialized nursing services like hospice.  The discharge process is a partnership between Nursing Case Management and Social Services.

Social Services
The hospital’s Social Services Department assesses patient needs, addresses mental health/behavioral health and substance abuse/dependency issues, and when needed, puts patients and their families in touch with the appropriate community resources. The Social Service Department provides crisis assessment services to the ED and clinic for those patients needing immediate assistance for psychiatric hospitalization services.
