What You Will Need Prior to Receiving Services

Prior to receiving services at Cibola General Hospital, you will need your written orders from your physician (these must include the diagnosis, test, and provider signature). You will need a photo identification card, insurance cards, and your Social Security number. Please be prepared to pay your co-pay. Some insurance companies also require a prior authorization. Also make sure to follow any specific pre-procedure instructions (such as abstaining from eating or drinking) given to you by your provider. Patients under 18 years of age should be accompanied by a parent or legal guardian.

Parking and Transportation

All parking at Cibola General Hospital is free. Additionally, the Cibola Transit Authority provides drop off to the hospital. Visit http://villageofmilan.com/departments/transit/index.php for more information on stops and times.

Visitor Information

Emergency Room
Visitors are limited to the primary waiting areas with the exception of one family member/friend. Visitors may be asked to leave the room when care is being provided. Children are not generally permitted in the ER rooms.

Labor & Delivery
Visitors are allowed in the labor/delivery rooms in accordance with hospital policy.

ICU
Visitors in this area are generally limited to up to two family members/friends at a time. The length of the visit depends on the patient’s condition and visitors may need to leave when care is being provided. Children must have their visits planned in advance with the nurse. Please note the ICU is closed to visitors during shift change.

Outpatient Surgery
Patients going into surgery are allowed two family visitors prior to their procedure regardless of time of day. Family remaining at the hospital during surgery should stay in the OR waiting area. Children must be supervised at all times

Limiting Visitation
If a patient chooses to limit visitors, a sign will be placed on the patient’s room door directing visitors to check in at the nurse’s station, otherwise, visitors are allowed per hospital policy. In most circumstances, children may visit in the hospital’s main visiting area and patient rooms as long as supervised by an adult.

Family Visitor Support
Friends and family play a vital role in the healing process. We encourage family members and friends to visit patients and participate in their care.

Registering for Your Outpatient Services, Procedures and Surgeries

There are two Registration Offices in the hospital, one located beside the Emergency Room (open 24 hours a day, seven days a week) and one near the Outpatient Entrance (open 7:30 AM – 4:30 PM Monday through Friday).

When you register, the admitting clerks will ask for information that is required by law, private insurance companies, and/or Medicare and Medicaid. All patient information is held in the strictest confidence. When registering, you are asked to bring any and all of the following documents that are applicable to you:
Active medical insurance cards
Photo identification
Written orders from your physician (including diagnosis, test, and provider signature)
Social Security number

It is the goal of Cibola General Hospital to make the registration process as convenient and as speedy as possible, though it should be understood that patients who need to be seen in the Emergency Room do have priority over patients registering for routine tests and procedures.

Your Financial Responsibilities

To maintain our ability to provide the best healthcare possible, the following payment expectations apply for non-emergency services for both insured and uninsured patients.

Your deductible, copayment and/or co-insurance, if applicable, are due at the time of service. A deductible is the amount you must pay before your benefit plan starts paying any portion of your medical services. After your deductible is met, you may have to pay a copayment (a set dollar amount) or co-insurance (a percentage of the cost) for your medical care. Some plans do not require a deductible. Please refer to your insurance plan for the amounts that you must pay and what is applicable to you. At the time of registration we will provide you with an estimation of total costs and require your estimated portion of the payment at that time. This is just an estimation and not the final bill. Any portion of your bill that your insurance company does not cover becomes your responsibility.

We do accept cash, checks, debit and credit cards. We provide a discount if you pay your estimation in full at time of service. If you are unable to pay on the day of your appointment, we may reschedule your appointment.

Financial Assistance: Financial assistance and counselors are available to our patients who are uninsured or underinsured, and have difficulty paying for care. Social Service Assistance/Assessment and Chaplaincy Program

Cibola General Hospital provides Social Services and Chaplain Services to assist with personal and spiritual needs.

Cafeteria

The cafeteria is designed to serve hospital employees, patients, and visitors. Visitors are welcome to eat in the cafeteria any time it is open. The cafeteria offers a food service line, snacks, chips, and a variety of hot and cold drinks/juices. Vending machines are also available by the radiology area.
