Healthcare for Our Community
The Health Center offers acute and chronic medical care, offered by quality providers. In addition, we offer preventative medicine services, such as routine physical exams, pap smears, immunizations, and early detection.
Making an Appointment
We strive to ensure medical care is convenient for you and your family. We ask that appointments are made in advance by calling during business hours: Monday through Friday, 8:00 AM to 5:00 PM.
We ask that you contact us if you will be late for your scheduled appointment or need to cancel.

Please note, should the Holiday fall on a Saturday, the offices will be closed the Friday before. If they holiday falls on a Sunday, the offices will be closed the Monday after.

- Acute Care: non-emergent illness, injury or exacerbation of a chronic condition.
- Nurse visits: visits requiring care that is limited to the scope of nursing duties to include immunizations or blood pressure checks.

Emergencies – If your health concern is critical, Cibola General Hospital is open 24/7, 365 days a year.
Phone Calls
Phone calls will be returned during the course of the day, as the schedule allows. Please have patience with us and remember that Providers and their Medical Assistants are seeing scheduled patients throughout the day, so it may take some time before your call can be returned. The office functions with an efficient and timely messaging system, so it is not necessary to repeat your call throughout the course of the day.
EMERGENCY situations and urgent appointment needs:
Frequently we are able to provide same day appointments for established patient in urgent care situations. If you are calling about urgent care or an emergency situation, please let the receptionist know immediately so your call can be handled appropriately. Please leave a detailed message if you reach our answering system. For all medical emergencies if you are unable to reach a receptionist, please call 911.

Receiving Additional Tests/Services
We refer you to Cibola General Hospital, or the most convenient location for you, to obtain radiology and laboratory services, if necessary.
Prescription Renewals
Our policy is to write prescriptions that are affordable, cost effective, and medically necessary. If you are in need of a prescription renewal, please call us at (505) 287-6500 as early as possible. We try to ensure your prescription renewal is available that same day, however, in some circumstances it will not be available until the following business day.
Questions about Your Bill
Our goal is to provide timely, cost-effective healthcare to you and your family. We make every effort to ensure we provide you with reasonably priced products and services. We do not purchase services for you as a “middle-man” retailer. Therefore, you will receive a bill directly from the laboratory, radiology, or other services you may have used.
If you have questions about your Explanation of Benefits (EOB) from your insurance company or from a statement we send you, please call our billing department at (505) 287-4446.
Insurance
Cibola Family Health Center accepts most health insurances, for example Presbyterian, Medicare, Medicaid, Tri-West, Blue Cross / Blue Shield, and most other major insurance plans.
Sliding Fee Scale
Cibola Family Health offers a sliding fee scale to our uninsured/under-insured patients. This program helps our patients afford quality healthcare. The sliding fee scale is based on the Federal Poverty Guidelines, which means household income plus number residing in household. If you are interested in applying for our sliding fee scale, please contact our business office. Be prepared to provide proof of income, tax returns, monthly pay stubs or wage statements, and photo ID.
